American Nurses Caregivers

Every member of Medical staff of American Nurse are pre-screened, evaluated and assessed of their education and skill before they are assigned to each individual patient.

• Each applicant is interviewed in person and criminal background check is submitted to the state.

Elder Care Coordinator

Primary day to day contact and central communication point for the family, doctor and American Nurses.

Case Manager

Receives and analyzes initial patient referral, coordinates with the doctor’s diagnosis, medication, care requirement and treatment goal; and determines the team to be assigned on the case. Executes and oversees the customized care plan and supervises the other caregivers in coordinating their visits and care of the patient’s case.

Registered Nurses (RN) and Mental Health Nurse

Performs the initial in-home assessment, reviews and recommends management of the formulated plan of care. Evaluates patient’s living condition and safety, reviews patient’s medication and any monitoring devices, defines the visit frequencies and prepares 485 evaluations and short-term goals.
